Fallopian tube patency assessment

Fallopian tube patency assessment

21 октября 2022 Обновлено: 03.11.2022
The fallopian tubes are the “paths” along which spermatozoa rush to the egg from the uterine cavity. They will successfully reach the target if the spaces inside the tubes are free. When the so-called adhesions (areas of dense connective tissue) become an obstacle to their movement, then they talk about tubal occlusion, which means infertility. Adhesive processes result from inflammation due to infection, or post-abortion curettages. Sometimes the cause of occlusion lies in the growth of endometrial polyps or myomas - they close the tube opening from the uterus.

Hysterosalpingosonography (sono-HSG) is a high-precision, minimally invasive method to detect tubal infertility and intrauterine abnormalities. It is an ultrasound-based examination, which is well performed in a liquid medium. Therefore, as a contrast, physiological sodium chloride solution is injected into the uterus and tubes to increase the image clarity.

Benefits of the hysterosalpingosonography

• affordability – it is performed in an ambulatory setting, at a doctor's appointment;
• informativity – a high image quality due to contrast;
• painlessness – only not quite pleasant pulling sensations in the lower abdomen are possible;
• therapeutic effect - sterile saline flushes the pipes from accumulated mucus, which facilitates the movement of spermatozoa in the future;
• ultrasound safety has been confirmed by decades of its medical use.

Contraindications

Despite the advantages, ultrasound contrast examination of the uterus and tubes is a serious manipulation, and it has certain contraindications. They are as follows:
• inflammatory diseases of the reproductive system;
• sexually transmitted infections;
• low degree of the vagina cleanliness according to the smear test (a large number of leukocytes, pathogenic flora);
• tumors of the pelvic organs.

Preparation

Sono-HSG is performed only after examination by a gynecologist and transvaginal ultrasound scanning of the urogenital system. Also, the doctor will prescribe some tests, first of all, a smear from the vaginal and cervical canal to determine the degree of purity of the excreted secret and to detect sexually transmitted infections. Approximately 30-40 minutes before the procedure, you need to drink at least a liter of fluid to completely fill the bladder.

Performance

Hysterosalpingosonography bears similarity to transvaginal ultrasound. It is performed in the first half of the menstrual cycle before the onset of ovulation to exclude a possible pregnancy. The doctor exposes the uterine cervix using a gynecological speculum and inserts a thin tube into the cervical canal - a Foley catheter, through which a contrast agent is injected under ultrasound control. First, the fluid is directed to the uterine corners, and after complete passage through the tubes, it is poured into the abdominal cavity. With tubal occlusion, its movement stops, which is noticeable either by expanding the uterine cavity, or by enlarged lumen of the tubes. After removal of the catheter, saline is poured into the vagina, and the rest of it is absorbed into the abdominal cavity through the peritoneum.
Tubal factor infertility accounts for almost half of the cases, so it is so important to undergo an examination on time. In EVACLINIC IVF, hysterosalpingosonography is performed by experienced professional specialists according to strict indications and after careful preparation.
